CNA

• $425.00 due upon registration (balance due end of second week)

• A valid state issued photo identification

• An official Social Security Card

• Negative PPD or Chest X-ray

• 17 years of age or older

• Criminal Background Check

• Navy Blue Scrubs, White Lab Coat, White Non-Skid Shoes

CCMA/PCT/EKG/Phlebotomy Technician

Applicants seeking admission to the *CCMA, PCT, EKG and or Phlebotomy Technician program must submit official records verifying the completion before the application can be processed:

• $500.00 due upon registration (balance due end of second week)

• *Current Georgia Nurse Aide Certification (Except for CCMA)

• A standard high school diploma from an accredited high school;

• GED (General Education Diploma), with the official test results;

• A signed affidavit submitted by the parent or legal guardian of a home schooled student attesting that the student has completed a home education program pursuant to the requirements of Georgia;

• A valid state issued photo identification

• An official Social Security card

• Criminal Background Check

• Negative PPD or Chest X-ray

• 18 years of age or older
